How to Sign Up to Reserve a Spot on a Team



 

Follow the instructions below to sign up for a trip. Then, if you haven't already, read the Mission Trip Basics, about personal experiences of past travelers with FHM, and the Getting Ready to Go section of this website.

  • Complete the REQUIRED forms (click to download and complete electronically).

  • Email your form to Justin Davis, FHM's mission team coordinator, if you have used the electronic signature.

Otherwise, mail both completed forms along with your $200 deposit (deposit is for regular teams; it is waived for medical relief teams in March) to:

  • Pay your $200 deposit by:

    • Mailing a check to the address above

    • Calling FHM with your credit card information (type of card, number and expiration date).  Our telephone is 919-382-5500. 

    • Submitting a credit or debit card payment on-line through PayPal.  (Make sure you mail us the forms.)



    *****Don't forget to make your plane reservation. Check with FHM first to make sure you will be on the right flight from Miami to Haiti and from Port-au-Prince back to Miami. See the Mission Guidebook for more details. *****

     

     

    Important Supplies for Medical Relief Teams going to Haiti in March and April

    Participants will be:

    • sleeping on the floor in the Blanchard Clinic
    • using bucket baths in the Blanchard lavatories
    • eating two meals a day at the Blanchard Clinic (breakfast with coffee and bread and sometimes hard boiled eggs; supper with rice and beans or spaghetti with chicken, hot dogs etc.)

    FHM will provide water purification tablets. We expect to have some Internet access in the evenings and some electricity for lights. We will have access to local women who will wash clothes for an extra fee.

    Therefore, IN ADDITION TO THE PACKING LIST in FHM's MISSION GUIDEBOOK, each participant should bring:

      • air mattress
      • sheet(s)
      • blanket if you want to sleep on the roof when it is not raining
      • mosquito net with duct tape and rope for hanging, depending on the type of net you bring
      • extra flashlights/batteries
      • bathing supplies, e.g. towel, wash cloth, soap, shampoo, etc.
      • bills in low denominations to purchase soda, beer, and to pay a local person to wash clothes
      • additional food for lunch and to supplement the meals provided, e.g. dried fruit, tuna fish, protein bars, peanut butter
      • personal medicines, e.g. antihistamines, decongestants, Cipro, sleeping aids

    Those traveling from RDU will be asked to carry two 50-pound bags of supplies, including medicines and baby formula. Space will be allotted for some personal items.

    Participants should also read the latest CDC information on traveling to Haiti.
